To counter rising costs, traditional managed healthcare plans have responded by either limiting access to providers through strict provider networks (like HMOs) or loose provider networks (like PPOs) or somewhere in between. Plans have also tried limiting treatment and coverage. They have tried low deductibles encouraging care and moved onto high deductible plans encouraging savings when care is avoided. There have been significant benefits realized from these methods, but not without significant stress and frustration for both employers and plan participants.
A total management approach would not only yield savings, but increase the value of the overall healthcare investment and reduce the risk of legal liabilities from limiting care.
